Initial Assessment & Consultation – In person (secure video conference or telephone may be an option)
Initial assessment and consultation is best if it can be in person, however, using video conference and/or telephone can be considered too. Initial sessions take approximately 75 to 80 minutes. During this time we will gather information about your history, what is going on for you now and what you most want for your future health and wellbeing. We will create a personal blueprint specifically created for your healthier behaviors. We will also work together to determine the frequency of follow-up sessions and which modality would be best to use for your individual needs. To download our assessment form click here.

Follow-up Consultations – In person (secure video conference or telephone may be an option)
Follow-up consultations take about 50 minutes. The timing and frequency of follow-up sessions depends upon your needs and the plan we have developed for you. During this time we will focus on the details of your goals and the plan we developed, offer support and strategies, and reevaluate your progress.
Email Questions and Communication
Email can be an option to answering your questions or to check in between sessions. We will develop this into your program as mutually agreed upon and bill based upon the dietitian’s time at our follow up rate.
Insurance
The Dietitians at Bay Area Nutrition are providers for a number of insurance companies, including the Santa Clara County Individual Practice Association (SCCIPA), Aetna PPO, Affinity, HealthNet PPO, Stanford Healthcare Alliance, United Healthcare PPO and Medicare. If you need medical nutrition therapy to manage diabetes, high cholesterol or blood pressure, food allergies, eating disorders or gastro-intestinal disorders (IBS, Chrons, Celiac disease) your medical insurance may reimburse you. It is best to get a referral from your medical doctor and to contact your insurance company to make sure that medical nutrition therapy procedure codes (CPT) 97802 & 97803, is a covered benefit. We recommend you get any authorization in writing from your insurance company, otherwise you may be held responsible for the charges. You can find more information here.
Cancellation Policy
48 hour notice is required to cancel or reschedule appointments. If your appointment is scheduled for 10 am on a Friday, be sure to call before 10 am on the preceding Wednesdayday to avoid having to pay for the session. Please call us at: (408) 370-7731 for any scheduling changes.
